The entry token printed below provides the first chance for readers to sign up to this year's Baby and Toddler of the Year.

By entering, your child could be in with the chance of picking up a share of the £1,000 prize fund.
And as part of the competition, the first 500 entrants to enter can take advantage of an offer at Cheeky Chimps Playzone in Scunthorpe.

The venue is offering an exclusive free, school term-time weekday trip to those quick to enter.
The smiling faces of North Lincolnshire's under-fives can be captured at our specially set up studio in Scunthorpe's The Foundry shopping centre from Monday, February 11.
The winner of this year's competition will receive a total of £550.
This includes the £400 first prize and £150 for winning their age category, while they will also get a print of their winning photo.
The three other age category winners will each pick up a £150 cash prize.
Jenny Holt, promotions co-ordinator for the Scunthorpe Telegraph, said: "It is great news to be able to launch this year's Baby and Toddler of the Year competition.
"It is a competition that always proves popular and a great chance for our readers to get involved.
"It is great to be teaming up with The Foundry shopping centre again.
"This always proves to be a popular location and we are looking forward to welcoming lots of smiling faces again this year."
Once photographs have been taken, parents or guardians will be given a date and time to return to the studio to view their photographs.
They will then choose one to be featured in the Telegraph on Thursday, March 21.
They will also have the opportunity to purchase the photographs.
After the photographs are featured in the newspaper, a voting process will begin to determine the finalists.
The age categories are 0-9 months, 10-18 months, 19-36 months and 3-5 years.
The age of the child must be correct from the last day of the photo sessions, which is Saturday, February 23.
This will determine what age category they fall into.
Peter Golightly, centre manager of The Foundry, said: "We are all delighted to be asked to host the competition once again as it is a fun way for families to have the chance to win some fantastic prizes.
"Last year there were a staggering number of entries and we hope that this year will be bigger than ever."
